Purpose of the Position:
The core purpose of the Jail Transition Coordinator is to connect incarcerated individuals, who are preparing for release or are likely to be released into the Clark County community, with essential resources. By establishing these connections, you will directly contribute to increasing desistance behaviors and reducing recidivism, fostering positive change in the lives of those you serve and enhancing the overall safety of our community. As a public servant, you will be dedicated to the safety and well-being of Clark County citizens.

Responsibilities:
As a Jail Transition Coordinator, you will be responsible for coordinating direct services for incarcerated individuals with acute and complex behavioral health needs within the Clark County Jail Reentry Program. Your duties will include, but are not limited to:
- Reviewing individual needs and barriers to successful reentry.
- Developing collaborative, individualized release plans in partnership with incarcerated individuals.
- Coordinating access to a wide range of community resources, including mental health, substance use support, basic needs, housing, healthcare, peer support, education, and employment services.
- Facilitating connections to trauma-focused services and other specialized support as needed.
- Working closely and collaboratively with corrections officers, jail medical and mental health staff, and community partner organizations to ensure seamless transitions.
- Effectively representing client needs with allied professionals and community organizations.
- Coordinating services across multiple agencies and systems to provide comprehensive support.
- Exercising professional judgment in making referrals and seeking guidance from program supervisors.
- Employing evidence-based strategies to encourage positive behavioral change in clients.
- Developing and maintaining accurate and timely written documentation.
- Ensuring the efficient and responsible use of public resources.
- Participating in community outreach efforts and activities as a representative of Clark County.
- Understanding and adhering to the established chain of command within the jail setting.

Education and Experience:- Required: A Master's degree in social work, counseling, mental health, behavioral sciences, or a related field from an accredited institution plus at least one (1) year of experience working with persons with legal system involvement OR A Bachelor's degree in social work, counseling, mental health, behavioral sciences, or a related field plus at least two (2) years of experience working with persons with legal system involvement OR 5 years of equivalent work experience.
- Preferred: Experience working with complex co-occurring disorders in adults; experience working in correctional facilities; and experience working with persons during incarceration.

Selection Process:The selection process will include an application review (pass/fail), a first interview (pass/fail based on 70%), a comprehensive background investigation (including a polygraph examination and drug test), and potentially a final selection interview. All offers of employment are contingent on successfully passing the background investigation, polygraph examination, and drug test.

Background Investigation Requirements:This position with Clark County Jail Services requires a comprehensive background investigation, which includes maintaining the highest moral character, integrity, honesty, truthfulness, impartiality, and ethical conduct. Candidates must also have a history of exceptional attendance.
Disqualifying factors may include, but are not limited to:- Felony convictions (some consideration for age/circumstance may be applicable)
- Domestic violence charges
- Manufacturing/distribution of illegal drugs
- Recent Reckless/Negligent/DUI (within the last 3 years)
- Recent illegal drug use (within the last 3 years)
- Dishonorable military discharge or failure to register for Selective Service
- Discharge from a Criminal Justice Agency related to honesty
- Affiliation with gangs or extremist groups
- Recent patterns of behavior eroding public trust
- Unsatisfactory employment history
- Falsifying application or personal history information
- Unsatisfactory personal or professional references
The background investigation process may include an in-person interview with an investigator. Candidates will be required to bring original documents such as their birth certificate, driver's license, social security card, and other relevant documents. Note Regarding Marijuana Use:Please be aware that even though medical marijuana possession/use may be legal under Washington State law, it remains unlawful under federal law. Clark County Jail Services does not distinguish between medical and illegal marijuana use, and recent use may be disqualifying.
