The Ambulatory Intensive Care Unit (AICU) Care Coordinator (MA) functions as the central member of the clinic patient team as both the medical assistant and health coach, directed by the AICU Summit Manager. The Care Codinator acts as the key point of contact for patients managing their chronic conditions and advises patients on goal setting and action plans as the primary focus of managing their disease. Works directly with patients and their caregivers to monitor their care. Identifies and facilitates completion of preventative and follow-up care as it relates to assigned patients.
The AICU is not an acute care team, but a highly trained, integrated primary care team supporting patients with advanced illness. The AICU is part of Old Town Clinic (OTC), a patient-centered primary care clinic providing integrated behavioral and medical health care to homeless and low-income patients in downtown Portland.
Schedule: Monday - Friday, 8:00am - 5:00pm
Location: Old Town Clinic (727 W Burnside Street, Portland, OR 97209)
Compensation: Minimum rate of $27.40 per hour but could increase depending on candidate's experience.
