Health & Wellness Liaison

Description:

This position is responsible for working closely with the Director of Mental Health, Personal Health Navigator, and other staff to identify strategies and programming to reduce barriers to healthcare and increase health knowledge for students, families, and staff by performing the following duties.

Essential Duties

  • Identifies and facilitates partnerships with community partners to bring interesting, culturally responsive health and wellness programming to POIC schools and programs.
  • Develops and oversees health and wellness initiatives across POIC, including programming for both participants and staff to increase health equity by reducing barriers to care.
  • Plans and coordinates health and wellness-related events such as health fairs, health career fairs, community fun runs, and others.
  • Establishes and maintains effective working relationships with community partners, and youth and families from a variety of backgrounds.
  • Conducts training and builds school and community support to deliver effective school-based health and wellness services to students and their families.
  • Collaborates with school staff, caregivers, and other involved providers.
  • Supports evaluation of health and mental health services and service delivery models using standardized measures and outcomes.
  • Provides 1:1 case management and system navigation for students, families, and participants as needed to support overflow workload from Personal Health Navigator.
  • Keeps accurate, confidential record of case notes documenting sessions and adheres to high professional and ethical standards.
  • Builds school and community support for delivery of mental health and health services to students and families.
  • Meets periodically with staff to obtain feedback for case management, while maintaining confidentiality.
  • Performs other duties as assigned.
Experience/Education

A bachelor's degree is required for this role. In addition to the education requirement, this position requires three years of related experience. An equivalent combination of education, training, and experience may be used to satisfy these requirements.
