Job Description & Requirements Specialty: Master of Social Work Discipline: Social Work Start Date: 06/08/2026 Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Home Health LMSW - East Portland OR and Surrounding Areas TRAVELER MUST HAVE BOTH OR / WA LICENSURE (WA lic can be in the application process) - Start Date: ASAP - Shift: 8hr Days (*This facility cannot accommodate 4x10s or 3x12s.*) - Ratios: 1:1 (*May visit up to 6 patients per day; 30 productivity points standard) - Years of Experience REQ: 1 YR of Skilled Home Health Experience - First-Timers Accepted: Yes - Certs REQ: BCLS - Locals Accepted: Yes; Considered "local" within 50-75 mile radius. - Pending License Accepted: Yes, traveler must have BOTH OR and WA licensure (WA license can be in the application process) Special Requirements - Traveler is expected to have their own transportation Facility requires primary series COVID vaccination in addition to one booster - Oasis experience is required.
