Eden Health - - Responsibilities: Provide in-home physical therapy services to patients under physician plan of care; Review caseload with supervising PT on a regular basis; Complete visit notes via Electronic Health Record (EHR) on every patient; Educate patients and families/caregivers about physical therapy modalities; Identify and mitigate environmental/safety/mobility risks in patients' homes
