The ideal candidate will work within a multidisciplinary team, focusing on providing comprehensive therapy services to the geriatric population through our Program for All-Inclusive Care for the Elderly (PACE). Key Responsibilities Conduct thorough assessments and develop individualized treatment plans for elderly patients. Monitor vital signs and assess pain levels in patients. Provide patient and family education regarding home exercise programs and the use of adaptive and assistive devices. Identify environmental barriers impacting patient care and recommend solutions. Documentation of patient progress and treatment outcomes in the electronic health record system (EPIC). Ensure the delivery of quality care in home health and skilled nursing facility settings. Qualifications 1 year of experience in outpatient therapy, specifically with the geriatric population. Current BLS certification required. Experience with wheelchair seating prescriptions is a plus. Proficiency in EPIC charting system preferred. Experience in long-term goal setting and time management is highly valued. Work Schedule The position requires a 4-day work week, Monday through Thursday, with 8-hour shifts from 08:00 to 16:30. No weekend shifts or on-call responsibilities are required. Additional Information Patient Age Groups: Geriatrics Patient Diagnoses: Frail geriatric Work settings will include a mix of home health and outpatient services. Travel experience is required for this position. Reliable transportation is necessary for home visits. Locals within a 50-mile radius are welcome to apply. Orientation and Training The successful candidate will undergo an 80-hour orientation program to ensure effective integration into the team and adherence to our standards of care.
