Job Description & Requirements Specialty: Physical Therapist Discipline: Therapy Start Date: 07/13/2026 Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Estimated Gross Weekly Pay: $ 2887.00 Opportunity: Home Health PT - West Portland OR and Surrounding Areas Job Details: • Work 8-hour day shifts (facility cannot accommodate 4x10s or 3x12s) • 1:1 patient ratio with up to 6 patients per day (30 productivity points standard) • No weekend requirements • No on-call responsibilities • No floating between units required • Block schedule not available • Guaranteed hours with facility able to call off up to one shift per pay period (every two weeks) • 1 year of skilled home health experience required • First-time travelers accepted • Oregon license required • BCLS certification required • OASIS experience required • Must have reliable transportation with occasional maximum radius of 60 miles per day • Business casual attire required, no jeans (scrubs allowed but not required) • Chart using EPIC Remote Client for Home Health on laptop and Rover phone app/OASIS • Free parking provided • IRS standard mileage reimbursement • Work with adults and geriatric patients with diagnoses including dementia, Parkinson's, ALS, MS, orthopedic post-op, and post-stroke conditions • Complete approximately 16 hours of non-billable modules pre-start and annually Facility: This home health organization provides comprehensive physical therapy services to patients in their homes throughout the Portland metro area and surrounding communities.
