Job Description & Requirements Specialty: Physical Therapist Discipline: Therapy Start Date: 07/13/2026 Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Job Summary: The position of Home Health Physical Therapist (PT) is based in Portland, Oregon, at a home health care facility. This role involves providing skilled physical therapy services to adult and geriatric patients in their homes. Candidates are required to bring their license on the first day of employment. The schedule consists of eight-hour daytime shifts, and the start date is 07/13/2026. Job Details: Location: Portland, Oregon Schedule: 8-hour days; minimum of 40 hours per week Required Qualifications: 1 year of skilled home health experience; valid BCLS certification; Oregon license; ability to work 40 hours per week; experience with EPIC electronic medical record system and Oasis documentation; valid driver's license and own transportation Preferred Qualifications: Experience with EPIC and Oasis systems, and previous home health experience Special Requirements: Traveler is expected to have their own transportation.
