Details Home health setting serving East Portland, OR and surrounding areas 1:1 patient ratio with up to 6 patient visits per day; standard productivity expectation of 30 points per day 8-hour day shifts, 5 days per week (4x10 and 3x12 schedules are not available) No weekend or on-call requirements Floating between facilities within the state of Oregon is required; refusal to float may result in assignment termination Travel radius of up to approximately 60 miles per day (roughly 60–90 minutes total drive time); system groups clients to minimize windshield time Charting via Epic Remote Client on laptop and Rover phone app; OASIS documentation also required Local candidates are accepted; candidates within a 50–75 mile radius are considered local and are eligible at the same rate Requirements Minimum 1 year of skilled home health PT experience required Current BLS certification required Epic EMR experience required OASIS experience required Valid PT license required; pending license accepted Must have reliable personal transportation Minimum of 2 supervisory references covering 1 year of employment within the last 3 years in a comparable specialty or setting SSN and DOB required for consideration License required if applicable Additional Information Business casual attire required; scrubs are permitted but not required, no specific color, no jeans Free parking available Mileage reimbursement provided at the IRS standard rate First-time travelers are welcome to apply Former permanent staff must wait at least 3 months before returning as a traveler and must not reside within 50 miles of the submitting facility Minimal RTO is preferred; any time-off requests should be disclosed upfront, as post-offer RTO requests may be denied American Traveler Job ID P-714656.
