Details Home health setting serving East Portland, OR and surrounding areas 1:1 patient ratio with visits to up to 6 patients per day; 30 productivity points is standard 8-hour day shifts, Monday–Friday (4x10 and 3x12 schedules cannot be accommodated) No weekend or on-call requirements Travelers are expected to float between all facilities within the state; refusal to float may result in termination of the assignment Charting is done via Epic Remote Client for Home Health (laptop) and Rover phone app/OASIS Candidates within a 50–75 mile radius of the facility are considered local and may be accepted at the same rate Requirements Minimum 1 year of skilled home health experience required Current BLS certification required Epic EMR experience required OASIS experience required Valid PT license required if applicable Must have reliable personal transportation Minimum of 2 supervisory references covering 1 year of employment within the last 3 years in a comparable specialty or setting Full SSN and DOB required for consideration Additional Information Business casual attire required; scrubs are allowed but not required and no specific color is mandated; jeans are not permitted The scheduling system groups clients to minimize drive time; occasional maximum daily radius of roughly 60 miles (60–90 minutes total drive time) Mileage is reimbursed at the IRS standard rate First-time travelers are welcome to apply Former permanent staff must wait at least 3 months before returning as a traveler and cannot be within 50 miles of the submitting facility Minimal RTO is preferred for this position; a small number of days may be approved American Traveler Job ID P-726457.
