Schedule - Shift: 8-hour day shifts

Assignment Details - Patient load: 1:1 ratio; may visit up to 6 patients per day

Requirements - Required: 1 year of skilled home health experience - Required certification: BCLS/BLS - Required skill: OASIS experience

Roles & Responsibilities - Provide case management and care coordination for home health patients. - Conduct discharge planning and patient/caregiver education. - Document consistently with the plan of care and coverage criteria. - Ensure knowledge of Medicare/Medicaid and related home/community-based services.

Compliance / Notes - Licensure: Must have both OR and WA licensure; pending licensure may be accepted (confirm with hiring manager). - Vaccination: Confirm COVID-19 vaccine requirements with hiring manager. - References: 2 references covering 1 year within the last 3 years required. - Parking: Free parking available. - Scrubs/Attire: Business casual preferred; scrubs allowed; no jeans.
